The surgery will contact you if there is a problem but it is best to contact us for all results as the doctor may have given advice based on your results anyway.

When you attend for a test of any kind you will be told how long you should expect to wait for the results. Please bear this in mind and only contact the surgery once sufficient time has elapsed.

- if applicable, the details of the person you are completing the form on behalf of
