To ensure we continue to prescribe medicines safely and in line with national clinical guidance, our practice is introducing a new Medication Review and Monitoring Policy.
Many medications require regular reviews and in some cases, essential monitoring such as blood tests, blood pressure checks or other clinical assessments. These reviews help us to confirm that your medication remains appropriate, effective and safe and allow us to identify any potential side effects or changes that may require adjustments to your treatment.
We have identified that a significant number of patients are currently overdue for their medication reviews and/or essential monitoring. A medication review is a formal review with a GP or Pharmacist which can be a telephone appointment. Without this information, it may not be safe or clinically appropriate for us to continue prescribing certain medications indefinitely.
As a result, we are implementing a new policy to improve patient safety. Patients who are due or overdue for a medication review or required monitoring will be contacted. Prescriptions may be issued for a limited period while these appointments are being arranged, but ongoing prescribing may be delayed or paused if the required review or monitoring is not completed within a reasonable timeframe, unless there are exceptional clinical circumstances.
We will be contacting patients via text/email to inform them when their monitoring or medication reviews are due. To avoid disruptions to your supply of medication, please arrange relevant bloods and appointments when contacted by the surgery.
Patients will be contacted 3 times over the course of 2 months. After the third reminder, the supply of medication will be reduced until these reviews are completed.
This policy has been introduced to support safe prescribing, reduce the risk of medication-related harm and ensure all patients receive the appropriate level of clinical care. We appreciate your understanding and cooperation in helping us maintain the highest standards of patient safety.
