Social Prescribing

Access a range of local community groups, free activities and support for your social, emotional or practical needs. You will be assigned a link worker who will work with you on a one to one basis to help find activities and support services that will improve your wellbeing.

Mill Street Medical Centre Social Prescribing service

Social Prescribing involves Link Workers working with patients to support and signpost to local community groups, activities and services that may benefit your general health and wellbeing.

The social prescribing programme is free to all patients of Mill Street Medical Centre.
